ADDITIONAL PAGES FOR U.S. PASSPORTS If your passport pages are completely filled with visas or airport stamps you do not have to apply for new passports. Instead, GenVisa can obtain additional pages for your U.S. passports. Supplemental pages can be added up to 3 times during the life of a passport. Please note that if the passport is close to expiration, you may be better off by applying for a passport renewal. This procedure should only be used to add visa pages to a previously issued, currently valid U.S. passport. Please have the following items send to GenVisa for processing: 1. Application form "DS-4085". The form MUST be filled and printed out online. Please click here for the form.
2. Your most recent passport as described above. 3. Letter of Authorization - please click here to download. 4. Travel itinerary - Copy of a plane ticket, or travel itinerary with the applicant's name showing dates and flight information for a departure within 14 days or 30 days in case you need a visa. 5. Payments:
- $142.00 Government fee, payable with a personal check to US Department of State for Expedited processing. Please indicate your DOB in the memo field. 6. Service fee, payable to Generations Visa Service.
- $70.00 for more than 11 business days processing.
- $110.00 for 5-10 business days processing.
- $150.00 for 1-4 business days processing.
